Here are the step-by-step on how to use this form

Step1: LOGIN CREDENTIAL - kindly fill-up the the username, password and site (where you are currently assigned) then press the LOGIN button.

After you've successfully login your account, you will lead to this form. In this form you will see all of your current assets.
In order to make a transfer, kindly choose what asset you need transfer or find it via FA CODE on the search bar at the upper left of the form. 

Step 2: After you chose/found the asset you will be transferring, kindly check the check box on the right side. (Note: You can only select the asset if the status is Accepted.

Step 3: After checking the check box, select the type of transfer you will gonna do.

In transferring asset within your department, click INTER DEPARTMENT TRANSFER.

If the transfer of asset is from other site to another site, click the INTER SITE TRANSFER.
If the transfer of asset is from company to another (for ex. RCSI to RCSNI or vice versa), click INTER COMPANY TRANSFER.

Step 4.1: INTERNAL DEPARTMENT TRANSFER - Fill-up the details needed

After you've been completed to fill-up the form. Press SEND

Step 4.2: INTER SITE TRANSFER - Fill-up the details needed

After you've been completed to fill-up the form. Press SEND

Step 4.3: INTER COMPANY TRANSFER - Fill-up the details needed

After you've been completed to fill-up the form. Press SEND

Once you already send the form, the asset you will need to be transfer will need to be approved by FAD

The approver will receive an email regarding for the pending transfer of asset.
